West Texas Pharmacy Association Scholarship

All students attending an accredited school of pharmacy are eligible to apply for a WTPA scholarship

Must be a current WTPA member in good standing. Dues for students are $10 and payable
online at http://www.westtexaspharmacy.org/membership-dues

• If member dues have not been paid when the application is submitted, the application
  will be considered incomplete. **

Personal Statement:
*A 200-500 word autobiographical essay in which you discuss significant
experiences, community involvement, and any qualities of character and leadership you
might possess. Highlight any personal accomplishments, achievements, and experiences
that have helped you form that character. Be sure to include any pharmacy organization
participation and extracurricular activities. We would like to hear what your educational
and career goals are. Finally, let us know what it would mean to you to receive a
scholarship from WTPA.

Letters of Recommendation:
*One must be from a pharmacist (i.e. mentor, employer, preceptor, member of WTPA,
etc.)
*Recommendation form is provided.

Letter of Good Standing:
*Contact the School of Pharmacy
